Having helped someone study for the NCLEX exam I recognised the question.  I would assume the OP is probably in nursing school somewhere.

You and she/he have established that she/he is suffering past-present-

…PATIENT NEEDS INCREASING DOSAGES

Whatever you are doing/prescribing is not sustaining her/his relief.

YOU… are her/his doctor. You pause to reflect. You are holistic… you consider the WHOLE situation. She is in a stable environment… her pain is not.

YOU do what YOU think is right… “…first, do no harm.”
