What jlazc said. In addition, when you wet the stain to clean it, it "re-wets" the stains since they are water soluble, and they spread even further or "come back". If there is surface mold involved, use 50/50 water and bleach in a spray bottle, spray it good and let it dry. Then prime with an oil based primer like Kilz, BIN, Cover Stain, etc. If you use a water based primer, it will "re-wet" the stain again and it could migrate through the primer before it can "seal" the stain.

If it is a long term wet issue and mold has "come through" the drywall from the back, then you need to replace it. When the drywall is removed, soak down the joists and such with same said 50/50 solution before the new drywall goes up.

Hopefully, you have smooth ceilings? Makes things much easier.
