He refers to Rubio as Little Marco which is an insult to shorter guys. He referred to Carly Florina during the debates as ugly. He recently called Adam Schiff Little Adam Schiff. 

Few years ago, he told an attorney who was taking a deposition from him that she was disgusting for attempting to take a prearranged and carefully-timed break for pumping breast milk.

 It starts at the top.  He needs to change his approach and others will follow. From the cheering, it appears that the majority of those who attend his rallies enjoy when he allows himself to get into the gutter with his personal attacks. Maybe, in time, he will change...but 70 year olds aren't known to change.
