both medically and politically.  He knows the nation needs his advice but he also knows if he gets on the wrong side of Trump he is as good gone.  He knows he has to first stroke Trumps ego by saying something he did was good in order to be able say what he really needs to say which is in complete contridiction of Trump, and he is smart enough to say it quickly while Trump is still distracted preening his feathers and congratulating himself for receiveing a nod from Fauci.
