to answer your other questions, instincts just kick in when something major goes wrong. The more a pilot has seen, the easier it is to handle the unknown and to stay calm and collected. Every year we spend several hours in the simulator practicing emergencies, and that training kicks in when things get bad. Handling the aircraft comes first, but we always try to talk to the folks in back if we have the time. Just like the commercial says......never let them see you sweat.
