I've been eating out in restaurants all my life (literally, my mother did not cook much) and I have never "demanded" service. I do expect that they understand the art of service since they have taken the job -- that is, gauging the kitchen, putting the orders in a timely manner, getting the order to the table while the food is still hot, and in the correct sequence; and checking back periodically.
The best service is friendly and seamless, not overbearing. Nor is it lackidaisal -- like forgetting to bring the salads, and expecting the customers to be okay with that.

And the restaurant manager should always be immediately available to deal with an unhappy customer. That's part of his job.

Unfortunately, I'll never be a good ole boy, and that's probably a good thing.
